This webinar focuses on integrating screening for SDOH in patient interactions and incorporating positive screens into prediabetes and diabetes care plans. Participants will gain insights into how these factors can disproportionately affect health outcomes, as well as the significance of connecting patients to community supports in pursuit of diabetes management and prevention.
